More than 12,850 specialized healthcare training positions are called for the year 2027 through the Order SND/854/2026, published in the BOE on August 10, 2026. This call activates the selective processes MIR, FIR, EIR, PIR and their equivalents for graduates in Medicine, Pharmacy, Nursing, Psychology, Chemistry, Biology and Physics who want to access a healthcare specialty recognized in the national health system.

The positions are distributed among accredited public and private centers in all autonomous communities. Participation in selective tests is exclusively electronic, with no possibility of in-person submission of applications.

What does this regulation establish?

Order SND/854/2026 simultaneously approves the offer of positions and the call for selective tests to access specialized healthcare training in 2027. It regulates seven different university qualifications, each with its own selective process and assigned number of positions.

The complete distribution of positions by qualification is as follows:

QualificationSelective processPositions called
MedicineMIR9,676
NursingEIR2,341
PharmacyFIR377
Clinical PsychologyPIR293
BiologyBIR69
PhysicsRFIR57
ChemistryQIR37

In addition to the distribution by qualification, the call establishes two special cross-cutting quotas:

  • Disability quota: 10% of positions in each qualification is reserved for people with recognized disabilities.
  • Quota without residence authorization: a specific quota is enabled for applicants who do not have residence authorization in Spain at the time of the call.

Economic and operational impact

For accredited healthcare centers—both public and private—this call means the incorporation of more than 12,850 new residents in 2027. Each position implies an employment relationship with regulated remuneration, mentoring, clinical supervision and training resources during the specialization period (between 2 and 5 years depending on the specialty).

For accredited private hospitals and health centers, teaching accreditation is a factor of competitive differentiation and talent attraction. The call activates position assignment processes that these centers must manage with their human resources and teaching teams.

For applicants, access to a resident position means the start of a paid employment relationship with the healthcare system, with remuneration set by agreement or regional regulations, during the entire training period.

Who does it affect?

  • Graduates and degree holders in Medicine who want to access any medical specialty via MIR.
  • Graduates and diploma holders in Nursing who opt for a nursing specialty via EIR.
  • Graduates and degree holders in Pharmacy who want to specialize via FIR.
  • Graduates and degree holders in Psychology who opt for the Clinical Psychology specialty via PIR.
  • Graduates in Biology, Physics and Chemistry who access specialties in the field of health sciences (BIR, RFIR, QIR).
  • Accredited public and private healthcare centers in all autonomous communities, which will receive and mentor the assigned residents.
  • People with recognized disabilities who opt for the reserved 10% quota.
  • Foreign applicants without residence authorization in Spain, who have a specific quota enabled by this call.

Practical example

A Medicine graduate who finished their degree in 2025 and wants to specialize in Cardiology must participate mandatorily in the MIR process called by Order SND/854/2026. They compete for one of the 9,676 available MIR positions. If they have a recognized disability degree equal to or greater than 33%, they can opt for the disability quota, which represents 10% of those 9,676 positions (approximately 967 positions reserved only for this group in Medicine). All procedures—registration, submission of documentation and position assignment—are carried out exclusively electronically, without travel to any office.

A private accredited hospital in Barcelona, for its part, must have its reception protocols, mentoring and remuneration ready for the residents assigned to it from 2027, depending on the accredited positions it has.

What should applicants do now?

  1. Verify compliance with qualification requirements: confirm that the university degree obtained (degree, degree or diploma) is included in the qualifications called by Order SND/854/2026.
  2. Prepare documentation in digital format: all participation is electronic, so it is essential to have access to a digital certificate or valid electronic identification system.
  3. Check if you are entitled to the disability quota: applicants with recognized disabilities should assess whether it is convenient for them to compete for the reserved quota (10% of positions) or for the general quota.
  4. Review the quota for applicants without residence authorization: foreign graduates without residence in Spain must identify the specific quota enabled for them and the specific requirements for participation.
  5. Consult the distribution of positions by autonomous community and center: positions are distributed among accredited public and private centers in all autonomous communities, which allows planning destination preferences.
  6. Follow official registration deadlines: the call has been published since August 10, 2026; the specific deadlines for submitting applications are included in the full text of Order SND/854/2026 in the BOE.
